Clinical Pharmacist

Job Title and Grade: Band 8 Clinical Pharmacist

Location/Trust: Bolton Hospital

Roles and Responsibility of the role:

· Clinical screening and accuracy checking of prescriptions.

·Supporting the safe and timely dispensing and supply of medicines, including discharge prescriptions.

· Providing medicines information and pharmaceutical advice to patients and healthcare professionals.

· Undertaking medicines reconciliation and clinical medication review on wards, where appropriately experienced and competent.

· Identifying and resolving medicines-related problems and supporting safe prescribing.

· Contributing flexibly to pharmacy services in response to operational pressures and service demand.

To apply for this post, you will need:

· MPharm (or equivalent overseas degree).

GPhC pharmacist registration.

Well-developed therapeutic and clinical pharmacy knowledge

Post qualification experience in hospital pharmacy including recent hospital pharmacy experience

Experience working as part of MDT
