MedicsPro is recruiting a Community Mental Health Practitioner to support an Older Adult Mental Health Team.

This role is suitable for a qualified mental health practitioner with experience working within a community Older Adult Mental Health or Memory Service setting. The successful candidate will manage a community caseload with support and supervision, undertake assessments of new referrals, and contribute to multi-disciplinary team working.

Essential Requirements

• Qualified mental health practitioner
• Experience working within a community Older Adult Mental Health Team or Memory Service
• Experience managing a caseload autonomously within a community setting
• Ability to assess new referrals to mental health services
• Experience working collaboratively within a multidisciplinary team environment
• Ability to communicate effectively with clients, carers and families via telephone and face-to-face contact

Key Responsibilities

• Manage a community caseload with support and supervision
• Assess new referrals received by the service
• Liaise with clients, carers and families through telephone and face-to-face contact
• Provide duty cover as required by the service
• Participate in regular multidisciplinary team meetings
• Support the delivery of community-based mental health interventions for older adults

Shifts and Working Arrangements

• Location: Harold Wood (Travel to other sites required)
• Service: Older Adult Mental Health Team
• Working pattern: Monday to Friday
• Hours: 09:00-17:00
• 30-minute unpaid break
