- 須具社會工作學位或以上之註冊社工，5年或以上長者/復康服務及3年管理經驗；具長者服務項目經驗可獲優先考慮
- 主要負責為受更換升降機影響之長者/殘疾人士提供綜合外展社會服務，包括：個案管理、樓梯機服務、膳食及生活物資送遞服務，及組織不同義工服務及宣傳性活動，並與不同持份者溝通協調﹔協助計劃主任完成項目相關要求
- 良好溝通、組織、管理能力、能獨立工作、團隊合作精神、熟悉電腦軟件應用 (包括Word、Excel)
- 工作地區：全港﹔辦公室：沙田/元朗
