申請人須持有有效之社工註冊證及認可之社會工作學位，在香港中學文憑考試五科考獲第 2 級或以上 (包括英國語文科及中國語文科)，或具同等學歷。負責識別及支援隱蔽及高風險長者、照顧者及其家庭成員。提供個案輔導、危機介入、外展服務、轉介跟進及協調社區資源；協助建立、維持及更新中央資料庫資料，運用服務數據進行需要評估、服務監察及成效分析；與社區伙伴合作；按服務需要或需負責督導工作。具三年或以上長者服務相關經驗，持有「安老服務統一評估機制評估員」資格者獲優先考慮。此職位屬時限性合約制，天水圍區長者鄰舍中心上班。
