Join our dedicated home healthcare team as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA) for a permanent home care case! In this vital role, you will provide compassionate, personalized care to clients in their own homes, supporting their health, safety, and well-being. You will be the key to enhancing quality of life for individuals with diverse needs, including elderly patients, those with disabilities, or individuals requiring specialized memory and dementia care. Your expertise in patient observation, assistance with activities of daily living (ADLs), and medical support will help clients maintain independence and comfort.

Responsibilities

• Deliver comprehensive personal care assistance, including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
• Monitor and record vital signs accurately; observe patient conditions closely to identify any changes or concerns.
• Assist with medication administration following prescribed protocols while maintaining strict adherence to home health care regulations and HIPAA confidentiality standards.
• Support clients with activities of daily living (ADLs), including meal preparation, light housekeeping, laundry, and cleaning to promote a safe and healthy environment.
• Document all patient interactions and medical information precisely using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems; ensure medical records are up-to-date and compliant.
• Provide specialized dementia and Alzheimer’s care, memory care, hospice support, and end-of-life assistance with compassion and professionalism.
• Communicate effectively with clients, families, and healthcare teams to coordinate care plans and ensure client satisfaction.
• Follow all safety protocols including first aid, CPR certification, and basic life support procedures to respond promptly during emergencies.

Requirements

• Valid CNA or HHA certification with current licensure as required by state law.
• Proven experience working with geriatric patients, individuals with disabilities, or in assisted living environments.
• Knowledge of medical terminology, catheter care, medication administration, and vital signs monitoring.
• Experience working within home health agencies or providing home & community care services preferred.
• Ability to perform heavy lifting safely; familiarity with patient transfer methods such as Hoyer lifts is advantageous.
• Strong understanding of home health care regulations, HIPAA compliance, and maintaining patient confidentiality.
• Excellent communication skills for effective client interaction and documentation accuracy.
• Certification in first aid, CPR, and basic life support is required; additional training in dementia or hospice care is a plus.
• Ability to work independently in a live-in employment setting or flexible schedules that meet client needs.

Pay: $24.82 - $29.89 per hour
