Community Case Manager

The pay for this position is $44,850-$58,9089 annually. $500.00 sign on bonus Sometimes it takes only one person to make a difference in a life. As the Community Case Manager for children’s services, you could be that person. The Community Case Manager works directly with individuals receiving services, connecting them with resources, documenting client progress and supporting follow-through on treatment plans and referrals

• An Associate’s Degree and two years experience and/or Bachelor’s Degree.
• The ability to establish and maintain supportive relationships with children, youth and families.
• Knowledge on how to provide holistic and therapeutic case management services.
• Experience working in the mental health field.
