Job description

Job Title: Community Connector - Grade 5

Location of post: Community based across Neath Port Talbot County Borough

Fixed Term until 30 September 2027

Workstyle: Your workstyle in this role will be a frontline/community based worker.

About the role:

We are currently seeking two person-centred, reliable and flexible individuals to join Fy Mywyd Independent Living Service as Community Connectors. This rewarding role supports adults with learning disabilities and/or neurodivergent conditions to build independence, develop skills, access community opportunities and achieve personal outcomes.

As a Community Connector, you will provide planned 1:1 and small-group support in community settings, in line with assessed needs, individual outcome-focused plans and risk assessments. You will help people develop independence, participate in their communities and build valued relationships and natural support networks.

Your daily routine will include:

  • Providing safe, reliable and person-centred 1:1 and small-group support.
  • Agreeing clear, achievable steps with individuals to support their personal outcomes.
  • Planning, delivering and evaluating individual and group activities.
  • Supporting independent travel and living skills, including shopping, meal preparation, looking after the home and managing money.
  • Supporting access to volunteering, learning, training and community opportunities.
  • Maintaining accurate records and reporting risks, safeguarding concerns or changes in need promptly.

About you:

Do you have:

At least one year of experience in health, social care or a related support setting.

Experience of providing person-centred 1:1 support to help an individual achieve agreed outcomes.

Experience of planning, delivering and evaluating activities for individuals and groups.

A relevant qualification, such as NVQ/QCF Level 3 in Health and Social Care or equivalent.

An understanding of safeguarding, positive risk enablement, professional boundaries, confidentiality and safe lone working.

The ability to communicate clearly, maintain accurate records, work independently and manage competing priorities.

The flexibility and ability to travel across the County Borough as required.

We would welcome those with experience in the following roles: support worker, community support worker, enablement worker, care worker or similar person-centred health and social care roles.

About us:

We believe in rewarding and recognising our colleagues’ efforts and achievements. We also believe that there’s life at work and life outside of work. We want everyone to be healthy and happy and have the financial resources and support they need.

Choose to work for us and you’ll be rewarded with a range of attractive benefits and support to help you live your best life.

  • Hybrid working (in some roles) and flexible working schemes to improve your work life balance
  • Benefit from a competitive employee package with a remarkable 15.3% employer pension contribution
  • Enjoy a holiday entitlement of 25 days, increasing to 32 days after 5 years’ service (not forgetting the additional 8 bank holidays too)
  • Access discounted gym membership with Celtic Leisure
  • Utilise the cycle to work scheme with up to 40% off bikes and cycling equipment
  • Purchase a new car through Tusker salary sacrifice scheme
  • Enjoy exclusive discounts at local and national retailers with the Smart Spend app through our staff wellbeing group.

If you would like to start a rewarding career with a company that values and invests in its employees and you have the necessary experience and qualifications, this could be your dream job and we would love to hear from you.
