Community Connector (OB Experience Preferred)

Role Overview: This position provides community-based advocacy and support for vulnerable members, serving as a local resource to assist with healthcare management and access to social services.

Key Responsibilities: Engage with members to support their healthcare needs, assist with scheduling, transportation, medication management, and follow-up. Support members in navigating health plan benefits and accessing community resources like housing, food, and employment. Conduct outreach to locate disconnected members, especially those with special needs, using data research and community visits. Guide members to maintain Medicaid eligibility and other financial resources. Participate in outreach activities, both telephonically and face-to-face, with significant local travel involved.

Qualifications & Skills: At least 1 year of healthcare experience, preferably with underserved populations; Community Health Worker (CHW) certification may be required depending on the state; valid driver's license and reliable transportation; strong multitasking, customer service, organizational, and communication skills; proficiency in Microsoft Office.

Preferred Attributes: Additional certifications like CMA, bilingual abilities, familiarity with healthcare systems, and knowledge of community-specific cultures are advantageous. The role demands adaptability, cultural awareness, and a commitment to community health.
