Community Health Worker I - FT, Days

This trusting relationship enables the worker to serve as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Hours/Schedule

Full-Time (40-hrs per week)

Days

Education: Minimum Required Qualifications High School Diploma or equivalent

Licensure: None

Experience: 1-year CHW experience or equivalent

Certification/Registration: Mercy provided Community Health Worker training certificate within 6-12 months

Other Skills And Knowledge: Strong relationship and collaboration skills

Excellent communication skills

Ability to work independently

Compassion for poor and underserved members of the community

Must be proficient with Microsoft Applications and be able to document in an EMR

Must have access to reliable transportation

Other Skills And Knowledge: Additional language skills as applicable to community
