About CareForMe
CareForMe is born from the conviction that the future of healthcare is not in the hospital but at home. We are a next generation home care provider looking to do things differently, to make sure that patients receive reliable and top quality care and clinical research, where and when they want. That’s why we embed technology in everything we do. We are now 1,300 nurses & practitioners strong in the UK. If you are passionate about making a real difference to patients and are ready to roll your sleeves up, please be part of our team!
About This Role
We are looking for a strong operator to take day-to-day ownership of our live community projects and see them through to their goals, from supporting hospital discharge to enabling clinical research access for patients
What You Will Do
- Run day-to-day coordination of CareForMe’s field activities across the UK
- Monitor field visits and respond to queries or escalations from patients and community workforce (nurses and support aides)
- Use the CareForMe platform to vet and train our community workforce to fill our service needs
- Communicate with patients and referral staff (e.g. NHS partners, other referrers)
- Assist in compliance workstreams, making sure visits are delivered safely and correctly
In practice, this role is roughly:
- 60% managing the nurse / support aide network
- 40% coordinating and overseeing patient visits and day-to-day operations
Who You Are
We gravitate heavily towards a can-do attitude and capability in our hiring decisions, not just years in post
- Care coordination experience or experience working with vulnerable populations (preferred)
- Registered nurse (preferred)
- Clinical research experience (preferred)
- Genuinely cares about patients and treats both patients and the community workforce as central to every decision
- SUPER organised and can handle hectic days calmly and effectively
- Extremely competent technology user
- Able to communicate clearly on the phone with patients and with community workforce
This Role Will Suit You If You Are
- A person who enjoys interacting with patients and community workforce on a daily basis
How to Apply
Please email your CV to [email protected] AND, briefly explain how you would handle these scenarios. 3 sentences max for each one.
- A practitioner has expressed interest in several visits, but each has been assigned to someone else. They tell you they feel overlooked and are becoming discouraged.
- A nurse has agreed to complete their onboarding but hasn’t done so despite several reminders.
- A patient who has a visit today has requested a female nurse, but only male nurses are available today.
