Pay: $25 per hour, plus approved business mileage reimbursement
Schedule: Approximately 12 hours per week, generally two weekdays; schedule agreed upon with management

About Warm Hearts Care

Warm Hearts Care provides non-medical in-home care, helping older adults remain comfortable and supported at home. Services include personal care, companionship, meal preparation, light housekeeping, errands, transportation, and support for individuals living with memory loss.

Position Overview

We are seeking a personable, dependable Community Outreach Representative to introduce Warm Hearts Care to local referral sources and help connect families with our private-pay services.

This position involves regular visits throughout King and Snohomish counties, relationship building, and consistent follow-up. It is ideal for someone who enjoys meeting people and supporting seniors and their families.

Responsibilities

• Visit senior communities, rehabilitation facilities, hospital discharge departments, senior centers, and other potential referral partners.
• Introduce Warm Hearts Care and explain our services, service area, and private-pay rates using approved materials.
• Deliver brochures, business cards, and rate sheets.
• Build relationships with social workers, discharge planners, care managers, and community professionals.
• Arrange introductory meetings and client assessment appointments for management.
• Follow up with contacts by telephone, email, and scheduled visits.
• Attend approved networking and community events.
• Maintain accurate records of visits, contacts, follow-up dates, and referral opportunities.
• Submit a weekly activity report and communicate inquiries promptly.
• Protect client privacy and represent the company professionally.

Qualifications

• Strong communication, relationship-building, and organizational skills.
• Comfortable introducing services in person and following up consistently.
• Ability to work independently and maintain accurate records.
• Reliable transportation for local visits; valid driver’s license and automobile insurance if driving for work.
• Ability to use email, a smartphone, and basic spreadsheets or contact-management tools.
• Home care, senior services, healthcare outreach, or sales experience preferred.
• Existing relationships with local referral sources are an advantage.

What Success Looks Like

Success will be measured through meaningful conversations with referral partners, consistent follow-up, meetings arranged, and qualified private-pay inquiries that lead to care assessments.

Compensation and Initial Review

The position pays $25 per hour for all hours worked, including required reporting, follow-up, and travel between work locations. Approved business mileage is reimbursed separately. An initial performance review will take place after 30 days.
