We are seeking an energetic and compassionate Community Resource Coordinator to serve as a vital link between our healthcare services and the community. This role involves coordinating social services and patient support to ensure seamless access to resources. The ideal candidate will possess strong communication skills, cultural competency, and experience working with individuals from diverse backgrounds. Duties

• Collaborate with healthcare teams to identify patient needs related to social determinants of health, including transportation, and food security
• Assist patients in navigating social services, community resources, and healthcare programs through intake assessments and client communication
• Maintain detailed documentation of resource referrals, and follow-up activities within EMR (Electronic Medical Record)
• Provide culturally sensitive support, working effectively with individuals from diverse backgrounds and ensuring compliance with HIPAA regulations
• Conduct outreach initiatives to educate the community about available health and social services
• Familiarity with managed care principles, health insurance policies, utilization review/management practices
• Excellent client communication skills with the ability to work effectively with individuals from diverse cultural background

Pay: $16.00 - $19.50 per hour
