Duties

• Conduct patient assessments and coordinate care plans tailored to individual needs, including those with mental health conditions, disabilities, or substance use disorders
• Facilitate intake processes and discharge planning, ensuring seamless transitions between inpatient, outpatient, and community services
• Collaborate with healthcare providers, social workers, and community organizations to coordinate resources and support services for clients
• Maintain accurate electronic health records (EHR/EHR) management in compliance with HIPAA regulations and medical terminology standards
• Provide behavioral health support through counseling, addiction counseling, or mental health interventions as appropriate
• Assist with diagnostic evaluations and clinical case management to monitor progress and adjust care strategies accordingly
• Educate clients and their families about health conditions, treatment options, and available community resources

Requirements

• Prior experience working in hospital environments such as Level I Trauma Centers, ICU units, or pediatric settings is highly desirable
• Demonstrated knowledge of hospital medicine, emergency medicine, acute care, and post-acute care practices
• Experience working with children, individuals with disabilities or substance use disorders, and diverse cultural backgrounds
• Proficiency in EMR/EHR systems, medical terminology, and electronic health records management
• Certification in Basic Life Support (BLS), CPR, and familiarity with HIPAA compliance standards
• Strong skills in patient service, client communication, task prioritization, and case management
• Ability to work effectively within multidisciplinary teams in inpatient or outpatient settings and manage complex patient needs
