Community Worker

About the role

This role involves providing practical, person-centred support to individuals with disability and psychosocial needs within their homes and local community. Based in Brisbane and Surrounding Areas - Northside, Wynnum, Ipswich & Logan. You will assist participants to strengthen life skills, maintain safe living environments and access meaningful activities that promote wellbeing and inclusion.

Key responsibilities

• Providing structured in-home support with daily living tasks and routines
• Supporting individuals with psychosocial disabilities, mental health, substance use or transitioning from incarceration
• Supporting skill development and independence within the home environment
• Assisting with community access, appointments and social participation
• Implementing individual support plans and behaviour support strategies
• Monitoring wellbeing and reporting changes in needs
• Liaising respectfully with families and informal supports
• Maintaining accurate documentation and progress notes

About you

• Qualification in Community Services, Social Work, Disability, or related field (or working toward) - desired
• Experience supporting people with disability or psychosocial needs
• Strong communication skills and professional boundaries
• Ability to work independently and follow structured support plans
• Current driver's licence and reliable vehicle
• NDIS Worker Screening Check (or willingness to obtain)
• Full working rights in Australia
