CHL operates multi‑state HUBs and collaborates with federal, state, philanthropic, academic, and private partners to advance measurable community health outcomes.

  • Prepare reports and monitor program outcomes.
  • Ensure compliance with HIPAA, CMS grant requirements, and CHL policies.

Required Qualifications

  • Bachelor's degree in Public Health, Social Work, Human Services, Healthcare Administration, or related field preferred; equivalent experience considered.
  • Minimum two (2) years of experience in community health, care coordination, patient navigation, public health, or social services.
  • Experience working with faith-based or community organizations preferred.
  • Knowledge of healthcare access, Medicaid/Marketplace enrollment, community resources, and Social Determinants of Health.
  • Experience coordinating multiple stakeholders and community partnerships.

Knowledge, Skills & Abilities

  • Exceptional relationship-building and interpersonal skills.
