Consultant Physical Therapy

Adult Day Health Center. is currently hiring License Physical Therapist Consultant in San Bernardino, CA!

Purpose: In order to restore or maintain maximum mobility, the physical therapy services shall provide:

1. Assessment of participants' mobility level, strength, range of motion, endurance, balance, gait, ability to transfer, coordination, posture, and pain level.

2. Treatment to relieve pain and maintain or restore functioning.

3. Training to achieve and maintain maximum functional performance.

Duties:

• Conduct an initial assessment and PT Quarterly/Reassessments (if receiving PT). Assessment will evaluate all ADHC participant' physical condition and functional status related to PT.
• Develop a person-centered treatment plan taking into consideration the health needs, the participant goals and family needs including problems and goals.
• Record, date and sign treatments monthly on PT maintenance Treatment flowsheet, including a narrative note documenting changes as appropriate.
• Participate in MDT meeting.
• Train the center RN(s) to supervise maintenance program.
• The physical therapist will work the number of hours necessary to meet the needs of the participants.

Qualifications

Must have - Physical Therapy License

An individual who is licensed by CA State Board of Licensed Physical Therapy.

• Proven experience as a Physical Therapist (PT).
• Excellent knowledge of pharmaceutical care methods and procedures
• Physical Therapist will work the minimum hours required by Section 54423 CAC. PT will work the hours required by the previous 3 months average daily attendance and more as indicated by the needs of the center participants.
• The Contractor shall maintain liability and malpractice insurance as appropriate in the minimum amounts of $200,000/$600,000.

Schedule:
When needed and meeting minimum 30-32hrs a month average . 2 to 3 times a week to the center.

Supplemental Pay: Pay is based on experience
