Coordinated Entry Specialist

Organization Description

The Housing & Homeless Coalition of Central New York (HHC) is a non-profit organization working to reducing, preventing, and ultimately ending homelessness in Central New York. The HHC acts as the Collaborative Applicant, Homeless Management Information System and Coordinated Entry Lead for the United States Department of Housing and Urban Development’s (HUD) Continuum of Care (CoC) for Onondaga, Cayuga, and Oswego counties.

Position Summary

The Coordinated Entry Specialist will operate housing support activities on behalf of the CoC’s Coordinated Entry System. The Coordinated Entry System assesses and prioritizes people experiencing homelessness for permanent housing placements. The HHC acts as the Coordinated Entry lead for the CoC with duties including but not limited to overseeing and training assessment protocols, completing assessments in the community with those experiencing homelessness, distribution of the coordinated entry list, and providing technical assistance to agencies on their matches.

The position will support and participate in all the above-mentioned activities.

Accountability: CoC Planning Coordinator

FLSA: Non-exempt, full time, 37.5 hours/week

Salary Range: $26/hr to $30/hr

Responsibilities

· Facilitate Coordinated Entry and CNY-VI assessment training

· Closely collaborate with internal team members to maintain a solid, accurate, and up to date Coordinated Entry prioritization list in Homeless Management Information System (HMIS)

· Conduct the CNY-VI assessment and make referrals to housing and service providers

· Enter and update client level data in HMIS

· Provide technical assistance to any new providers entering into the Coordinated Entry system

· Assist with updating the HHC’s Coordinated Entry Policies and Procedures Manual with input from the Coordinated Entry Committee

· Ensure that CoC and ESG-funded agencies are following the policies and procedures from the manual

· Oversee Case Conferencing and Cross Sector Collaboration

· Provide documentation and client eligibility support to shelters and housing providers

· Review Stability Voucher (SV) applications and obtain required documentation

· Other duties as assigned

Qualifications

· Bachelor’s Degree preferred and experience working in housing and homelessness programming.

· Experience working in homelessness response system, including Coordinated Entry preferred.

· Experience and demonstrated skills in organization and management of multiple projects simultaneously with the ability to set priorities and meet deadlines.

· Proven ability to work with individuals from all levels and backgrounds; recognizes and values diversity; process information with consideration for cultural sensitivities.

· Proficient with MS Windows Office Suite; must have the ability to learn new and/or customized software.

· Ability to work with flexibility and independence.

· Ability to work overtime if necessary.

Pay: $26.00 - $30.00 per hour
