Role Description

The DCW/Caregiver is a full-time, on-site role based in Philadelphia, PA, providing direct in-home support to patients. Day-to-day responsibilities include assisting with activities of daily living such as bathing, dressing, toileting, grooming, and mobility, while monitoring the patient’s general well-being. The caregiver helps with light housekeeping, meal preparation, and errands to support a safe and comfortable home environment. The role also involves providing companionship, engaging patients in conversation and simple activities, and following care plans and agency protocols. DCW/Caregivers are expected to document services provided, report changes in condition to supervisors, and collaborate with the care team to ensure consistent, high-quality care.

Qualifications

• Ability to provide hands-on personal care, including assistance with bathing, dressing, toileting, grooming, and safe mobility support.
• Skills in household support such as light housekeeping, meal preparation, and basic home safety awareness.
• Strong interpersonal and communication skills to build trust, offer companionship, and interact effectively with patients, families, and staff.
• Reliability in following care plans, documenting services accurately, and reporting changes in patient condition to supervisors.
• Demonstrated compassion, patience, and respect for individuals of diverse backgrounds, ages, and health conditions.
• Ability to work independently in the patient’s home, manage time well, and maintain professional boundaries.
• High school diploma or equivalent preferred; prior experience as a caregiver, DCW, home health aide, or in a similar role is beneficial.
• Willingness to complete required training, meet state/DCW certification or registry requirements as applicable, and comply with background checks.
• Physical ability to perform caregiving tasks, including lifting or supporting patients as needed, with or without reasonable accommodation.
