Job Summary

We are seeking a dedicated and energetic Dietary professional to join our healthcare team, providing essential nutritional support and guidance to patients across various care settings. In this role, you will develop personalized nutrition plans, collaborate with healthcare professionals, and deliver nutrition counseling to promote optimal health outcomes. Duties

• Assess patients’ nutritional status through detailed evaluations, including medical history and dietary habits
• Develop and implement individualized care plans that address specific dietary needs and health goals
• Provide nutrition counseling utilizing motivational interviewing techniques to encourage positive behavioral changes
• Collaborate closely with physicians, nurses, and other healthcare professionals to coordinate nutrition support strategies
• Manage and document patient progress using electronic health records (EHR) systems accurately and efficiently
• Educate patients on nutrition support options such as tube feeding, weight management, and chronic disease management
• Conduct diagnostic evaluations and nutritional analysis to inform tailored interventions for diverse populations including geriatrics and patients in long-term or acute care settings

Experience

• Proven experience working with individuals with specific dietary needs, including medical nutrition therapy (MNT) in hospital or long-term care environments
• Supervising experience in a dietary or clinical nutrition setting is highly desirable
• Strong knowledge of physiology, clinical nutrition principles, and nutrition support techniques such as tube feeding
• Hands-on experience with EMR/EHR systems for documentation and patient record management
• Background in managing care plans for chronic conditions like diabetes, cardiovascular diseases, or weight loss programs
• Experience working with diverse populations including geriatrics and patients with complex medical conditions
• Familiarity with diagnostic evaluation processes, nutritional analysis, and the development of care strategies for acute or chronic health issues
