Direct Care Worker

Job Overview
We are seeking a compassionate and dedicated Direct Care Worker to provide exceptional support and care to individuals with diverse needs. Duties

• Assist individuals with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Support residents with medication administration and monitor vital signs as needed
• Help with meal preparation, feeding, and maintaining proper nutrition for clients
• Provide companionship and emotional support to reduce feelings of loneliness or isolation
• Operate medical equipment such as Hoyer lifts or wheelchairs safely and effectively
• Maintain accurate documentation within Electronic Medical Record (EMR) systems and update care plans regularly
• Engage in light housekeeping duties like cleaning client spaces to ensure a safe and hygienic environment
• Support clients with memory care needs, including dementia or Alzheimer's care, using behavior management techniques
• Assist with transportation or driving clients to appointments while adhering to safety protocols
• Collaborate with healthcare teams on case management and develop personalized care strategies

Requirements

• Prior experience in assisted living, nursing home care, or home health care preferred
• Knowledge of medical terminology, HIPAA regulations, and patient confidentiality standards
• Ability to perform heavy lifting and physical tasks safely using equipment like Hoyer lifts or transfer belts
• Strong communication skills for conflict management and social work interactions
• Valid driver’s license for transportation duties; clean driving record preferred
• Certification in first aid and CPR required; additional certifications such as hospice care or working with individuals with autism are a plus
• Ability to work flexible hours including evenings, weekends, or overnight shifts as needed
• Experience working with individuals with developmental disabilities or dementia is highly desirable
• Demonstrated ability to follow care plans accurately while providing compassionate support
