Responsibilities

• Provide personal care assistance including bathing, grooming, dressing, and mobility support to residents with varying needs.
• Assist with activities of daily living (ADLs) such as eating, toileting, and transferring using appropriate manual handling and patient transfer methods like Hoyer lifts.
• Monitor residents’ vital signs and observe for any changes in health or behavior, documenting findings accurately in care plans.
• Support medication administration following HIPAA guidelines and prescribed schedules while ensuring safety protocols are maintained.
• Prepare nutritious meals and assist with meal consumption to promote health and well-being.
• Maintain a clean, safe environment by performing light cleaning duties and adhering to infection control standards.
• Facilitate engaging activities tailored to individual interests, including memory care exercises for residents with dementia or Alzheimer’s disease.
• Support residents’ rights by respecting their preferences, privacy, and autonomy during all interactions.
• Assist with transportation needs within the community or to medical appointments using personal transportation or facility vehicles.
• Collaborate effectively with nursing staff and other healthcare professionals to implement care plans for residents with complex conditions such as neurodevelopmental disorders or developmental disabilities.

Skills

• Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments.
• Strong knowledge of patient observation techniques, dementia care, Alzheimer’s care, and memory care strategies.
• Ability to perform patient monitoring accurately and respond swiftly to emergencies through CPR and first aid certification.
• Skilled in assisting with ADLs using manual handling tools like Hoyer lifts and patient transfer methods; capable of heavy lifting when necessary.
• Excellent communication skills for engaging with clients respectfully while supporting their resident’s rights and personal preferences.
• Familiarity with HIPAA regulations ensuring confidentiality of patient information at all times.
• Experience working with individuals with autism, intellectual disabilities, or neurodevelopmental disorders; supporting their unique needs effectively.
• Competence in meal preparation, medication administration, vital signs measurement, and basic nursing tasks aligned with long-term care standards.
• Ability to work independently or as part of a team in fast-paced environments that require patience, compassion, and adaptability.
