Duties

• Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, and personal hygiene to promote comfort and independence
• Monitor patient health by taking vital signs, observing for changes, and reporting findings accurately to healthcare professionals
• Support medication administration following care plans and HIPAA regulations to ensure safety and compliance
• Prepare nutritious meals aligned with dietary needs and assist with meal consumption as needed
• Use patient transfer methods including Hoyer lifts and manual handling techniques to ensure safe movement and positioning
• Maintain clean living environments by performing light cleaning duties and adhering to infection control protocols
• Support residents during outings or appointments by providing personal transportation assistance and ensuring their safety

Skills

• Proven experience working with individuals with developmental disabilities, autism, dementia, or neurodevelopmental disorders in group home or assisted living settings
• Strong knowledge of patient observation techniques, dementia care, Alzheimer’s care, memory care, and resident rights
• Competence in using patient monitoring tools such as vital signs measurement devices and Hoyer lifts for safe patient transfers
• Familiarity with care plans, medication administration procedures, first aid, CPR certification, and HIPAA compliance standards
• Ability to perform heavy lifting safely while providing personal support or assisting with mobility needs
• Excellent communication skills for engaging with residents empathetically and collaborating effectively with healthcare teams
• Experience in home health or hospice care environments is highly valued for delivering compassionate support across various settings
