Responsibilities

• Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene.
• Monitor patients’ health by observing vital signs, medication administration, and overall condition, ensuring safety and well-being.
• Support residents with mobility and transfer methods using equipment such as Hoyer lifts and patient transfer techniques.
• Prepare nutritious meals aligned with care plans and assist residents during meal times to promote proper nutrition.
• Maintain accurate documentation of care provided, including care plans, patient observations, and medication records, adhering to HIPAA regulations.
• Engage in patient observation to identify changes in behavior or health status, communicating findings promptly to healthcare professionals.
• Support residents’ participation in community activities or outings, providing personal transportation assistance when needed.
• Follow infection control protocols through cleaning duties and maintaining a safe environment for residents and staff.
• Assist with specialized care needs such as dementia care, Alzheimer’s care, memory care, hospice support, or working with individuals with autism or developmental disabilities.

Requirements

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
• Knowledge of patient monitoring techniques including vital signs measurement and medication administration.
• Ability to perform manual handling tasks and heavy lifting safely while supporting resident mobility.
• Certification in CPR and First Aid is required; additional certifications such as HIPAA compliance are advantageous.
• Experience working with individuals with neurodevelopmental disorders or disabilities is a plus.
• Strong communication skills for effective client interaction and team collaboration.
• Demonstrated ability to follow care plans meticulously while maintaining confidentiality and professionalism.
• Flexibility to work various shifts including evenings, weekends, or holidays as needed to ensure continuous resident support.
