Responsibilities

• Assist residents with activities of daily living (ADLs), including personal care, grooming, dressing, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods.
• Monitor residents’ health by taking vital signs, observing patient conditions, and reporting changes to healthcare teams promptly.
• Support medication administration and ensure compliance with care plans while maintaining HIPAA confidentiality standards.
• Prepare nutritious meals, assist with meal consumption, and ensure proper sanitation and cleanliness in the living environment.
• Provide companionship and engage residents in meaningful activities to promote mental stimulation and emotional well-being.
• Maintain accurate documentation of patient care activities, incident reports, and progress notes in accordance with facility policies.
• Assist with transportation needs for appointments or community outings, ensuring safety during transfers and travel.

Requirements

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly preferred.
• Knowledge of dementia care, Alzheimer’s care, memory care, autism support, developmental disabilities, intellectual disabilities, and neurodevelopmental disorders is advantageous.
• Certification in CPR and First Aid is required; additional certifications such as Hoyer lift operation or medication administration are a plus.
• Ability to perform heavy lifting safely and assist with patient transfer methods while adhering to manual handling protocols.
• Strong communication skills to effectively interact with clients, families, and healthcare professionals.
• Demonstrated patience, compassion, and the ability to observe patient needs carefully for effective patient monitoring.
• Experience working within HIPAA guidelines to protect client privacy is essential.
• Flexibility to work various shifts including evenings, weekends, or holidays as needed.
