Duties

• Assist individuals with activities of daily living (ADLs), including personal care, grooming, dressing, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods.
• Monitor patient health by taking vital signs, observing for changes in condition, and reporting findings accurately while maintaining HIPAA compliance to protect privacy.
• Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation to promote healthy living.
• Support medication administration following care plans and ensure proper documentation of all medication-related activities.
• Provide transportation assistance for appointments, community outings, or errands to foster independence and community engagement.
• Conduct patient observation to identify safety concerns or behavioral changes, especially when working with individuals with autism, dementia, Alzheimer’s disease, or neurodevelopmental disorders.
• Maintain a clean and safe environment by performing light housekeeping duties such as cleaning common areas and personal spaces in accordance with health standards.
• Document care activities thoroughly in client records while respecting confidentiality standards like HIPAA.
• Engage in ongoing training on topics such as hospice care, geriatrics, social work principles, and patient monitoring techniques to enhance your caregiving skills.

Experience

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
• Demonstrated ability to provide patient personal care assistance with compassion and professionalism.
• Experience working with individuals with developmental disabilities or neurodevelopmental disorders such as autism or intellectual disabilities support is advantageous.
• Knowledge of care plans development and implementation tailored to individual needs.
• Familiarity with medical procedures including vital signs measurement, CPR certification, first aid response, medication administration, and manual handling techniques like heavy lifting or the use of Hoyer lifts.
• Background in dementia care or memory care settings enhances your ability to deliver specialized support for residents with Alzheimer’s disease or other cognitive impairments.
• Strong communication skills for effective client interaction and collaboration with healthcare teams while adhering to privacy standards like HIPAA.
• Ability to handle physically demanding tasks involving patient transfer methods safely while maintaining a caring attitude.
