Responsibilities

• Assist residents with Activities of Daily Living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods.
• Monitor residents’ health by observing patient conditions, and reporting changes promptly to healthcare teams or supervisors.
• Prepare nutritious meals, assist with meal consumption, and support dietary needs aligned with individual care plans.
• Support residents during community outings or appointments by providing personal transportation assistance and ensuring safety at all times.
• Maintain a clean and safe environment through regular cleaning duties, laundry services, and adherence to infection control protocols.
• Document care activities accurately in resident records, including patient monitoring notes, medication administration logs, and incident reports.

Requirements

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
• Knowledge of dementia care, Alzheimer’s care, memory care, autism support, intellectual disabilities support, and neurodevelopmental disorders is advantageous.
• Ability to observe patients closely for changes in condition or behavior while maintaining patient rights and dignity at all times.
• Strong communication skills for effective interaction with residents, families, team members, and healthcare professionals.
• Physical ability to perform heavy lifting, assist with patient transfers using proper techniques, and stand or walk for extended periods.
• Familiarity with HIPAA regulations ensures confidentiality of all resident information.
