Duties

• Assist individuals with activities of daily living (ADLs), including personal care, grooming, and hygiene to ensure comfort and dignity.
• Support residents with medication administration, vital signs monitoring, and adherence to care plans to promote health and safety.
• Engage in patient observation and monitoring to identify changes in condition or behavior, especially for those with dementia, Alzheimer’s, or neurodevelopmental disorders.
• Help with meal preparation and feeding, ensuring nutritional needs are met while respecting dietary restrictions.
• Operate patient transfer methods such as Hoyer lifts and manual handling techniques safely to assist with mobility and prevent injury.
• Maintain accurate documentation of patient progress, incident reports, and communication logs while adhering to HIPAA privacy regulations.
• Provide transportation assistance for appointments or community outings, fostering social engagement and independence.

Qualifications

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly valued.
• Knowledge of dementia care, Alzheimer’s care, memory care, and hospice care practices is preferred.
• Demonstrated ability to observe patients carefully and respond promptly to their needs; experience with patient monitoring is essential.
• Familiarity with care equipment such as Hoyer lifts, blood pressure cuffs, thermometers, and other patient monitoring devices.
• Strong communication skills for effective client interaction and collaboration with team members; experience working with individuals with disabilities or developmental disabilities is a plus.
• Ability to perform physical tasks including heavy lifting, manual handling, and assisting with transfers safely.
• Certification in CPR and First Aid is required; additional training in HIPAA compliance and medication administration is beneficial.
