Duties

• Assist clients with daily activities such as bathing, dressing, grooming, and mobility to enhance comfort and independence
• Provide companionship and emotional support to reduce feelings of loneliness and promote mental well-being
• Monitor health status, report changes, and coordinate with healthcare professionals as needed
• Administer medication reminders and assist with light household tasks to maintain a safe living environment
• Support clients with meal preparation and nutritional needs tailored to individual preferences and dietary requirements
• Follow care plans meticulously, ensuring all services are delivered safely and effectively
• Maintain detailed documentation of care provided, client progress, and any incidents or concerns

Qualifications

• Prior experience in home care or related settings is preferred; experience with Level II trauma centers or hospital medicine is a plus
• Knowledge of emergency medicine procedures and post-acute care practices enhances your ability to respond effectively in urgent situations
• Strong interpersonal skills with the ability to build trusting relationships with clients from diverse backgrounds
• Ability to handle physically demanding tasks such as lifting or assisting with mobility safely
• Certification in CPR/First Aid is highly desirable; additional training in geriatrics or medical-surgical care is advantageous
• Demonstrated reliability, compassion, and commitment to providing exceptional patient-centered care
• Experience working within teaching hospitals or outpatient clinics adds valuable insight into varied healthcare environments
