Responsibilities

• Assist residents with activities of daily living (ADLs), including personal hygiene, grooming, dressing, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods.
• Monitor residents’ health by taking vital signs, observing patient conditions, and reporting changes to healthcare professionals promptly.
• Prepare nutritious meals and assist with meal planning while ensuring dietary restrictions are followed according to care plans.
• Support residents with medication administration, ensuring proper documentation in compliance with HIPAA regulations.
• Engage residents in meaningful activities that promote mental stimulation and social interaction, including memory care and dementia care initiatives.
• Maintain a clean and safe environment by performing regular cleaning duties and adhering to infection control protocols.
• Provide transportation assistance for appointments or community outings, ensuring safety during transfers and travel.

Experience

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
• Demonstrated experience in patient observation, monitoring vital signs, and administering medication is essential.
• Knowledge of dementia care, Alzheimer’s care, hospice care, and memory care practices is beneficial.
• Familiarity with patient transfer methods such as Hoyer lifts and patient personal care assistance is required.
• Experience working with individuals with autism or developmental disabilities supports success in this role.
• Certification in CPR and first aid is necessary; additional certifications related to HIPAA compliance or heavy lifting are advantageous.
• Proven ability to communicate effectively with clients, families, and healthcare teams while respecting residents’ rights is critical for success.
