Direct Support Professional

At Champion Providers, we are seeking energetic and compassionate Direct Support Professionals to provide high‑quality care and support to individuals with intellectual and developmental disabilities on 1st and 2nd shift. As a Direct Support Professional, you play an essential role in each individual’s daily life and personal independence journey. You will assist with everyday living needs while promoting independence, advocacy, dignity, and a safe, nurturing environment.

Responsibilities

• Assist individuals with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and other personal care needs, while maintaining required documentation
• Perform household tasks such as grocery shopping, meal preparation, and maintaining a clean and safe living environment
• Assist with scheduling appointments and providing transportation, including accompanying individuals to community outings and social activities
• Monitor and recognize behavioral or physical changes and report concerns appropriately
• Administer medications according to prescribed care plans while strictly adhering to HIPAA privacy and confidentiality standards
• Demonstrate physical stamina to safely assist with tasks such as lifting or repositioning individuals

Qualifications

• Must be at least 18 years of age
• High school diploma or equivalent required
• Valid Ohio driver’s license, current auto insurance, and a motor vehicle record that meets Authorized Driver criteria
• Reliable personal vehicle available for transporting individuals served, preferred
• Knowledge of emergency response procedures and CPR/First Aid
• Ability to work flexible schedules, including evenings, weekends, and overnight shifts as needed, preferred
• Ability to read, understand, and implement Individual Service Plans, along with accurate service documentation
• Ability to push, lift, or carry up to 50 pounds
