Duties

• Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and personal hygiene to promote dignity and independence.
• Monitor patients’ health by taking vital signs, observing changes in condition, and reporting concerns promptly to healthcare teams.
• Support medication administration following care plans and HIPAA regulations to ensure safety and compliance.
• Prepare nutritious meals and assist with meal planning to meet dietary needs and preferences.
• Operate patient transfer methods including Hoyer lifts and manual handling techniques to ensure safe mobility.
• Maintain a clean, safe environment by performing light cleaning duties and adhering to infection control protocols.
• Document care activities accurately in client records, update care plans regularly, and communicate effectively with team members about client progress.
• Provide transportation assistance for appointments or community outings while ensuring client safety at all times.
• Support clients during community outings or home visits with personal transportation assistance.
• Engage in patient observation to recognize behavioral or health changes early, especially for individuals with Alzheimer’s, dementia, autism, or neurodevelopmental disorders.

Requirements

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
• Knowledge of patient monitoring techniques such as vital signs measurement and patient observation is essential.
• Experience working with individuals with disabilities—including developmental disabilities, autism spectrum disorder, or neurodevelopmental disorders—is highly desirable.
• Ability to perform heavy lifting safely using proper manual handling techniques and equipment like Hoyer lifts when necessary.
• Certification in CPR and First Aid is required; ongoing training in medication administration and HIPAA compliance is a plus.
• Strong communication skills to effectively interact with clients, families, and healthcare teams while respecting residents’ rights.
• Ability to perform light cleaning duties and meal preparation while maintaining a safe environment.
• Valid driver’s license and reliable transportation for client outings or appointments are preferred.

Pay: $16.00 - $18.00 per hour
