Direct Support Professionals provide home and community-based services to people with disabilities. Forward Horizons is a grassroots non-profit organization founded in April 2019 to support people with disabilities in Ashland and Bayfield counties.

Direct Support Professionals enjoy a lot of variety within a work week and appreciate this rewarding work of helping our communities to be more accessible to all. Direct Support Professionals must be compassionate, attentive, and flexible. To be a Direct Support Professional, prior experience working with people with disabilities is a plus, though is not required.

Requirements

• Genuine interest in helping others
• Professional demeanor
• At least 18 years of age with valid driver’s license (preferred). Access to reliable personal transportation (i.e. have your own vehicle) is a plus.
• Must pass the Wisconsin DOJ Caregiver Background Check

Duties and Responsibilities

• Timesheet recording via QuickBooks TimeTracker - training provided
• Sandata Electronic Visit Verification (EVV) for services as required – training provided
• Attending meetings and writing reports, if needed
• Communicating effectively with Forward Horizons Program Directors and Program Administrators to keep up on regular schedule changes and needs, and to stay informed on any inter-agency announcements or trainings
• Keeping client’s personal information confidential

Examples of tasks

• Supporting clients (adults and/or children) as they work towards their goals
• Acting as a role model for making community connections
• Providing transportation, as needed
• Assisting with personal cares, if needed
• Empowering clients to express their needs and preferences in a socially sustainable way
• Maintain positive interactions and relationships with clients

******How do we determine starting wage? 3 or more years verified experience in this field within the last 3 years, and an interview that confirms the applicant is a good fit for the role, means the candidate would start between 16.50 and $17.00 if hired. No direct experience in this field at any time and an interview that confirms the applicant is a good fit for the role, means the candidate would start at $15.00 if hired. Some experience? Or experience occurred longer than three years ago? Starting wage would be somewhere in between $15.00 and $16.50.
