Responsibilities

• Assist residents with activities of daily living (ADLs), including personal care, grooming, dressing, and mobility support using proper patient transfer methods and manual handling techniques such as Hoyer lifts.
• Monitor residents’ health by taking vital signs, observing patient conditions, and reporting any changes promptly to the healthcare team.
• Administer medications following care plans and ensure compliance with HIPAA regulations to protect patient confidentiality.
• Support residents during meals by preparing nutritious meals, assisting with eating, and ensuring dietary guidelines are followed.
• Maintain a clean and safe environment by performing routine cleaning tasks, laundry, and sanitation of common areas in accordance with health standards.
• Document care activities accurately in resident records, including patient monitoring notes, medication administration logs, and incident reports.
• Provide transportation assistance for appointments or community outings while ensuring safety during transfers using appropriate patient transfer methods.

Requirements

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
• Knowledge of dementia care, Alzheimer’s care, memory care, and neurodevelopmental disorders such as autism is advantageous.
• Demonstrated ability to observe patients carefully through patient observation skills and monitor vital signs effectively.
• Certification in CPR and First Aid is required; Nurse Assistant Registered required: additional certifications like Hoyer lift training or medication administration are a plus.
• Strong communication skills for engaging with residents respectfully and collaborating with healthcare teams.
• Ability to perform heavy lifting safely and assist with manual handling tasks required for patient transfers.
• Familiarity with resident’s rights, HIPAA compliance, and long-term care standards ensures ethical and legal adherence.
