Disability & Leave Forms Officer

Scale Up Philippines is looking for a detail-oriented Disability & Leave Forms Officer to support a US healthcare client with disability, leave, and Workers’ Compensation documentation.

Engagement: Project-Based
Start Date: ASAP

Key Responsibilities

• Process FMLA, STD, LTD, ADA/Accommodation, Return-to-Work, APS, and Workers’ Compensation forms
• Review medical records, physician notes, treatment plans, restrictions, and recovery dates
• Prepare forms for provider review and signature
• Track deadlines and follow up on incomplete or pending documentation
• Maintain accurate case trackers and submit completed forms through approved systems
• Coordinate with patients, providers, insurance carriers, and disability administrators
• Ensure accuracy, confidentiality, and HIPAA compliance

Qualifications

• Experience in healthcare administration, medical records, disability processing, insurance, or Workers’ Compensation
• Familiarity with FMLA, STD, LTD, ADA, Return-to-Work, or Workers’ Compensation documentation
• Strong medical terminology and documentation review skills
• Excellent attention to detail and deadline management
• Experience with EHR/EMR systems is preferred
• Experience with Sedgwick, Hartford, Unum, MetLife, Matrix, Aflac, Liberty Mutual, Sun Life, or similar carriers is an advantage
