Responsibilities

• Assist clients with daily activities such as personal hygiene, meal preparation, medication administration, and mobility support
• Develop and implement individualized care plans tailored to each client’s unique needs and preferences
• Provide transportation and driving support for appointments, errands, or outings as needed
• Monitor client health status, document progress, and communicate updates to healthcare teams or case managers
• Engage in light housekeeping tasks to maintain a safe and clean living environment
• Collaborate with healthcare professionals to ensure comprehensive patient care and adherence to medical protocols

Skills

• Proven experience in care home, assisted living, or home care environments with a focus
• Knowledge of medical terminology, medication administration, and basic nursing practices
• Certifications in first aid and CPR are highly desirable
• Strong understanding of behavior management techniques and case management principles
• Ability to perform heavy lifting safely when assisting clients with mobility or transfers
• Excellent communication skills to effectively interact with clients, families, and healthcare providers
• Valid driver’s license with reliable transportation for driving duties
