Disability Support Worker

About the role

The Disability Support Worker works under the supervision of the Registered Nurse and/or Manager-Client Services Division at Im With You. This role involves providing comprehensive, personalised support to NDIS participants and their families through individual and group activities, assisting with daily living tasks, implementing positive behaviour support plans, facilitating social activities, and supporting learning opportunities in both home and community environments. Working within Im With You's unique model of care, this position utilises digital client profiles, personalised training, and a collaborative healthcare approach to deliver exceptional person-centred support that promotes independence and community participation.

Key responsibilities

• Provide day-to-day support to NDIS participants and their families as per roster and individual support plans
• Support participants in age-appropriate activities and tasks, ensuring safe planning and implementation
• Assist clients with all aspects of daily living in home environments including personal care, domestic tasks, and community access
• Implement positive behaviour support plans under professional guidance from registered nurses and allied health professionals
• Support participants to access community activities and outings with direct care plans
• Deliver person-centred care that respects client preferences, choices, and individual goals
• Access and utilise Im With You's secure digital client profiles to review medication information, equipment manuals, and care instructions
• Update digital records with relevant observations and support notes
• Participate in the development of individual support plans and monitor client progress
• Work collaboratively within a multi-disciplinary team including registered nurses, physiotherapists, occupational therapists, and other healthcare professionals

About you

• Minimum Certificate III in Individual Support (Disability) or currently working towards this qualification
• First Aid Qualification (current)
• CPR certification (current)
• Valid driver's licence and reliable vehicle with comprehensive insurance
• Working with Children Check (current)
• National Police Check (within the last three months)
• NDIS Worker Screening Check (current)
• Current vaccination certificate (including COVID-19 as per health directives)
• Proof of right to work in Australia/Visa
• Passion for working with NDIS participants and their families and ability to relate well with vulnerable people, minority groups, and people with disabilities
• Professional, ethical, and empathetic approach when working with participants and families
• Clear, personable, and professional communication skills (written and spoken)
• Exceptional attention to detail and good computer skills for practice management software and digital client profile systems
• Ability to work both within a team and autonomously and capacity to work flexible hours including evenings, weekends, and public holidays as rostered
• Physical capability to perform manual handling and personal care tasks
• Punctual, reliable, and committed to service delivery with professional communication style
• Empathetic and compassionate approach to client care, flexible and adaptable to changing client needs
