District Nurse (Developing Case Manager considered)

Lincolnshire Community Health Services are seeking a full time District Nurse to join them within the Gainsborough Community Nursing Team. Applicants should hold the Community Specialist Practitioner qualification and have experience of managing a caseload of patients and leading a team of staff in delivering excellent patient care. There is an opportunity for this post to be a development role for a Case Manager on a fixed term basis of 1 year to allow for competencies to be obtained. This is a Fixed term post for 1 year to cover case managers post while they are training.

The service delivers care to patients in a variety of community settings across a 7 day service covering the hours of 8:00- 18:30 (shift work involved).

The District Nurse/Case Manager will hold a caseload of patients with varying health needs, coordinating care delivery. They will need to have extensive communication, interpersonal and clinical skills in order to be successful.
