Responsibilities

• Assist clients with activities of daily living (ADLs), including personal care, grooming, bathing, dressing, and mobility support.
• Monitor patient health by taking vital signs, observing for changes, and reporting findings accurately in compliance with HIPAA regulations.
• Support medication administration and ensure proper documentation according to care plans and medication protocols.
• Facilitate meal preparation and assist with eating to meet nutritional needs while respecting dietary restrictions.
• Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control standards are met.
• Provide transportation assistance for appointments, outings, or community activities to promote social engagement and independence.
• Follow individualized care plans tailored to each client’s needs, including dementia care, Alzheimer’s care, memory care, hospice care, or autism support.
• Engage in patient observation and communication to monitor behavioral changes or health concerns proactively.
• Collaborate with healthcare professionals, social workers, families, and team members to deliver comprehensive support aligned with each client’s goals.

Skills

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
• Strong knowledge of patient monitoring techniques including vital signs measurement and dementia or Alzheimer’s care strategies.
• Familiarity with HIPAA regulations ensuring confidentiality of client information at all times.
• Ability to perform patient transfer methods safely using Hoyer lifts or other assistive devices; manual handling skills are essential.
• Experience supporting individuals with neurodevelopmental disorders such as autism or developmental disabilities is advantageous.
• Excellent communication skills for effective client interaction and team collaboration.
• Certification in CPR and first aid is required; additional training in hospice care or geriatrics is a plus.
• Capable of heavy lifting when necessary while maintaining proper body mechanics for safety.
• Knowledge of medication administration procedures following prescribed protocols.
• Demonstrated patience, empathy, and a positive attitude when working with diverse populations across various age groups.

Pay: $16.00 - $21.00 per hour
