Job Description:

Make a meaningful difference in the lives of Arkansans.

Location: Remote/Field-Based (Candidates must reside in Arkansas)
Department: Enhanced Care Coordination
Reports To: Enhanced Care Supervisor
Employment Status: Full-time
FLSA Status: Non-exempt
Safety-Sensitive Designation: This position is designated as safety-sensitive. Essential duties include driving and other responsibilities requiring sustained alertness, sound judgment, and safe coordination. Employees must be able to perform safety-sensitive functions at all times and may be subject to drug and alcohol testing and/or fitness-for-duty evaluations in accordance with company policy and applicable law.

Our Shared Future

At Arkansas Provider Coalition (APC), we believe effective care coordination goes beyond completing paperwork. Our Enhanced Care Coordinators serve as advocates, problem-solvers, resource navigators, and trusted partners for members with complex healthcare and support needs.

As an Enhanced Care Coordinator, you will help members navigate complex systems, overcome barriers to care, and access the services and resources necessary to support their health, safety, and independence. You will work closely with members, families, guardians, healthcare providers, community organizations, and other members of the member’s support circle to coordinate services and promote continuity of care.

This role focuses on members with complex circumstances, including rapid hospital readmissions, guardian changes, juvenile system involvement, court-ordered treatment, and other situations requiring enhanced coordination and support. Assigned populations may include Tier IV members, individuals in therapeutic communities, re-entry populations, members involved with the Department of Children and Family Services (DCFS), and individuals with behavioral health needs, intellectual and developmental disabilities, substance use disorders, or other specialized needs.

If you have experience navigating complex healthcare systems, coordinating services, supporting vulnerable populations, and advocating for individuals and families, we encourage you to apply.

You and Your Job

As an Enhanced Care Coordinator, you will develop, implement, and monitor Person-Centered Service Plans (PCSPs) for assigned members designated as complex cases. You will assess member needs, identify barriers to healthcare access, coordinate appropriate services, and work collaboratively with members and their support circles to promote positive outcomes.

You will also support care transitions, communicate emerging risks, participate in care team discussions, and help ensure members receive appropriate, timely, and coordinated services. This position requires strong communication, organization, problem-solving, and time-management skills, along with the ability to manage a field-based schedule, travel throughout Arkansas, maintain accurate documentation, and respond effectively to changing member needs.

Job Responsibilities

Person-Centered Care Coordination

  • Develop, implement, document, evaluate, and monitor Person-Centered Service Plans (PCSPs) for assigned members with complex care coordination needs.
  • Coordinate services and supports based on member preferences, identified needs, goals, strengths, and individual circumstances.
  • Identify barriers to healthcare access and work collaboratively with members, families, guardians, providers, and other stakeholders to develop appropriate solutions.
  • Coordinate services across physical health, behavioral health, intellectual and developmental disabilities, substance use treatment, community resources, and other applicable service systems.
  • Assist members and their support circles in navigating healthcare systems and accessing appropriate services.
  • Provide information, recommendations, and support to help ensure the effective implementation of each member’s PCSP.
  • Monitor changes in member circumstances and communicate relevant information to support timely adjustments to care and services.

Member & Family Engagement

  • Maintain regular communication with assigned members, families, guardians, providers, and other individuals involved in the member’s support circle.
  • Conduct weekly calls with care teams and relevant stakeholders to discuss member needs, service coordination, progress, and potential barriers.
  • Facilitate communication among members and their support circles to promote continuity of care and coordinated service delivery.
  • Participate in discussions and meetings concerning assigned members, including care planning, case staffing, and care transition activities.
  • Provide education, guidance, advocacy, and assistance to help members and their families navigate available services and resources.
  • Identify and communicate risks, concerns, and changes in member circumstances that may affect health, safety, or service delivery.
  • Support members during transitions involving hospitalizations, changes in guardianship, changes in living arrangements, juvenile system involvement, re-entry, and other complex circumstances.
  • Maintain appropriate professional boundaries while building respectful, collaborative relationships with members and their support networks.

Collaboration & Care Coordination

  • Collaborate with healthcare providers, behavioral health professionals, community organizations, state agencies, families, guardians, and other members of the care team.
  • Coordinate care activities for complex populations, including Tier IV members, therapeutic community participants, re-entry populations, members involved with DCFS, and individuals receiving court-ordered treatment.
  • Assist with cases that have not yet been assigned when needed to support seamless care transitions and continuity of services.
  • Communicate necessary information, identify emerging risks, and escalate concerns to the appropriate internal or external stakeholders.
  • Participate in care team meetings, case discussions, and other activities necessary to support member needs and PCSP implementation.
  • Help resolve service coordination challenges and identify opportunities to improve communication, access to care, and member outcomes.
  • Develop or contribute to materials, resources, and processes that improve care coordination efficiency and reduce risks.

Documentation, Compliance & Quality

  • Maintain accurate and timely documentation of member contacts, care coordination activities, service planning, referrals, and follow-up activities in the designated electronic medical record (EMR) or other required systems.
  • Ensure documentation is completed in accordance with APC policies, contractual requirements, established timeframes, and applicable state and federal regulations.
  • Maintain the confidentiality, privacy, and security of member information and Protected Health Information (PHI).
  • Follow applicable HIPAA requirements and organizational policies regarding information security and data confidentiality.
  • Monitor assigned cases and communicate concerns that may affect service delivery, member safety, or compliance.
  • Remain informed about applicable care coordination requirements, community resources, and regulatory expectations.
  • Identify opportunities to improve workflows, reduce risks, strengthen care transitions, and support positive member outcomes.
  • Participate in quality improvement activities, training, and team initiatives as assigned.

Technology & Field-Based Work

  • Utilize company-issued technology, Microsoft 365 applications, communication platforms, the EMR, and other required systems to complete essential job functions.
  • Maintain a secure home office with reliable internet access and appropriate safeguards for confidential member information.
  • Schedule and manage appointments, member contacts, care team meetings, and required follow-up activities across an assigned caseload.
  • Travel throughout Arkansas to member homes, provider offices, community locations, meetings, trainings, and other assigned locations.
  • Effectively manage a field-based schedule while balancing member needs, travel, documentation, appointments, and regulatory deadlines.
  • Maintain company equipment and protect organizational assets in accordance with APC policies.
  • Demonstrate sound judgment, sustained alertness, and safe practices while driving and performing other safety-sensitive responsibilities.
