Responsibilities:

• Provide direct patient care to our residents, including medication administration, wound care, and vital sign monitoring
• Develop and implement individualized care plans to meet the unique needs of each resident
• Collaborate with other members of the healthcare team to ensure seamless care transitions and effective communication
• Demonstrate a commitment to excellence in care delivery, adhering to established standards and protocols
• Participate in quality improvement initiatives to continuously improve resident outcomes and satisfaction

Requirements:

• Current LPN license in the state of Massachusetts
• Completion of an accredited LPN program
• Strong clinical and communication skills
• Ability to work effectively in a fast-paced environment
• Compassionate and empathetic approach to care
