As an Executive, Eldercare Services (Contract / Community Settings), you will be responsible for the following duties:

• Drive the implementation of the programmes and workplans, including integrated care initiatives, joint referral workflows, and care coordination activities.
• Conduct interRAI assessments and support care planning and coordination for clients, where required.
• Build and manage strong partnerships with healthcare, community, and social service organisations within assigned sub-regions to facilitate service integration.
• Represent the organisation in the sub-regional initiatives and serve as a key liaison between internal teams, external partners, and community stakeholders.
• Coordinate the meetings and operational activities, monitor programme performance and progress, and support the resolution of service or operational gaps.
• Identify opportunities to improve service integration and quality, while supporting management reporting, dashboards, presentations, and programme oversight.
• Any other ad hoc duties as assigned.

Requirements

• Possess a Degree in Social Work, Nursing or healthcare-related discipline
