Responsibilities

• Deliver patient personal care assistance, including helping with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using manual handling techniques like Hoyer lifts and patient transfer methods.
• Monitor residents’ vital signs and observe patient conditions to ensure safety and well-being; promptly report any changes or concerns to healthcare professionals.
• Assist with medication administration following HIPAA guidelines and care plans to ensure accurate and safe delivery of treatments.
• Support residents with meal preparation, feeding, and nutrition needs while maintaining a clean and hygienic environment through regular cleaning duties.
• Provide specialized dementia care, Alzheimer’s care, memory care, hospice care, or support for individuals with neurodevelopmental disorders or developmental disabilities.
• Facilitate effective communication with clients and their families regarding care plans, resident rights, and progress updates.
• Use patient monitoring techniques to ensure safety during patient transfers or assisted living activities; utilize equipment such as Hoyer lifts when necessary.
• Support residents in community outings or personal transportation needs while ensuring safety and comfort during travel.

Skills

• Proven experience working with individuals with disabilities, intellectual disabilities support, or senior care in settings such as group homes or nursing facilities.
• Strong knowledge of patient observation techniques, dementia care strategies, and memory care best practices.
• Ability to perform heavy lifting safely and utilize manual handling equipment like Hoyer lifts effectively.
• Familiarity with medication administration procedures and maintaining accurate documentation in compliance with HIPAA regulations.
• Excellent communication skills for engaging with residents, families, healthcare teams, and colleagues professionally and compassionately.
• Competence in first aid and CPR certification to respond swiftly during emergencies.
• Experience working with individuals with autism or neurodevelopmental disorders is highly desirable.
• Ability to follow detailed care plans while demonstrating patience, empathy, and respect for resident’s rights.
