Experienced Support Coordinator - North Jersey

Disability Navigators LLC seeks an experienced Support Coordinator to serve our individuals and families in North and Central NJ.

When & Where will you be Working?

We are seeking an energetic, experienced, part-time Support Coordinator to join our dynamic team immediately. This is a part-time position, with the potential to grow to a full-time position over time. While you will mostly be working from home, we are looking for a Central or North Jersey resident, with the ability to travel up to 90 minutes from your home for visits with individuals and their providers.

Responsibilities:

• Engage adults with intellectual and developmental disabilities and their families, fully understanding and documenting who they are and what supports they need.
• Schedule and facilitate planning team meetings with family, providers, and other supporters, in collaboration with the individual.
• Notify the individual, planning team, and service provider of ISP revisions when services are changed, reduced, or services are terminated.
• Ensure delivery of quality services, in a safe manner, within full consideration of the individual’s rights by conducting monthly monitoring, quarterly face-to-face visits basis, and annual home visits.
• Documenting all contacts with the individual, their family, and their providers.
• Understand and comply with all DDD and Medicaid rules.

Requirements:

• Bachelor's degree from an accredited university in any discipline
• Minimum two years experience as a Support Coordinator in New Jersey
• One or more years of experience working with individuals with intellectual and/or developmental disabilities (This experience can include paid employment, volunteer experience, and/or being a family caregiver of an individual with an I/DD.)
• Strong oral and written communication skills
• Attention to detail
• Ability to manage multiple tasks (and their follow-up) effectively
• Hold a valid driver’s license
• Complete and pass background check and fingerprinting

Compensation and Job Type

• Starts at $18,540 and grows with caseload
• Start at part-time; can grow to full-time as caseload expands
