Family Nurse Practitioner

Job Summary
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The FNP will provide comprehensive care to patients across various age groups, focusing on health promotion, disease prevention, and management of chronic conditions. The ideal candidate will have experience in outpatient settings. Full time and part time positions are available.

Duties

• Conduct thorough physical examinations and assessments of patients.
• Develop and implement individualized care plans based on patient needs.
• Provide health coaching to promote wellness and preventive care.
• Manage cases for patients with complex health issues, including infectious diseases.
• Utilize aseptic techniques in all procedures to ensure patient safety.
• Maintain accurate patient records using MDland software.
• Advocate for all patients, ensuring they receive appropriate care and resources.

Skills

• Proficiency in conducting physical examinations and health assessments.
• Strong knowledge of case management practices and health coaching techniques.
• Experience working in outpatient settings.
• Familiarity with aseptic techniques and infection control protocols.
• Ability to manage infectious disease care effectively.
• Competence in using MDland or similar electronic health record systems.
• Excellent communication skills, both verbal and written, to interact effectively with patients and healthcare professionals.
• Strong organizational skills with attention to detail in documentation and patient care processes.
