Responsibilities

• Conduct comprehensive patient assessments, including vital signs and physiological evaluations, to determine care needs.
• Manage patient care plans by collaborating with physicians, therapists, and other healthcare team members to develop personalized treatment strategies.
• Provide clinical case management across various settings such as home health, outpatient clinics, and inpatient facilities.
• Document patient interactions accurately using Electronic Health Records (EHR) systems while ensuring compliance with HIPAA regulations.
• Assist with discharge planning by coordinating post-acute care services and educating patients on medication management and health maintenance.
• Perform diagnostic evaluations and monitor patient progress during ongoing treatment episodes.
• Support infection control protocols and maintain a safe environment for patients with infectious diseases or requiring specialized care such as hospice or behavioral health.

Experience

• Proven clinical experience in critical care environments such as ICU, Emergency Room, or Level I/II trauma centers.
• Extensive hospital experience including medical-surgical units, pediatrics (PICU), geriatrics, and post-acute care settings.
• Demonstrated expertise in managing complex cases involving trauma, infectious diseases, or behavioral health conditions.
• Familiarity with hospital medicine practices, discharge procedures, and utilization management processes.
• Proficiency in medical terminology, vital signs assessment, and physiology knowledge to support accurate patient evaluation.
• Experience working with Electronic Health Records (EHR/EHR) systems and adhering to health regulation policies like HIPAA.
• Ability to collaborate effectively with healthcare professionals across disciplines to ensure comprehensive patient service delivery.
