FULL TIME 3-11 LICENSED PRACTICAL NURSE

Overview

Join our dedicated healthcare team as a Full-Time 3-11 Licensed Practical Nurse (LPN) and make a meaningful difference in the lives of our residents.

Duties

• Deliver nursing care to residents in long term care, assisted living setting.
• Administer medications
• Conduct resident assessments by monitoring vital signs, performing physical examinations, and evaluating patient conditions.
• Document all patient interactions accurately in Electronic Health Records (EHR) systems while maintaining strict compliance with HIPAA and confidentiality policies.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient's needs.
• Provide education and support to patients and their families regarding health management, dementia care, Alzheimer's care, pain management, and medication administration.
• Communicate with outside services as necessary (MDs, PT/OT, Speech)

Requirements

• Valid state licensure as a Licensed Practical Nurse (LPN)
• Experience working with geriatric patients, including memory care, dementia care, or hospice experience preferred, but not mandatory.
• Demonstrated ability to perform physical examinations, vital signs assessment, specimen collection/processing, and basic life support (BLS).
• Strong communication skills for caregiver interaction and patient education across diverse populations.
• Ability to work efficiently during evening hours (3 PM – 11 PM) while maintaining attention to detail in a fast-paced environment.

Pay: From $35.00 per hour

Expected hours: No less than 40.0 per week
