Full-Time Hospice RN

Role Overview:

This position involves evaluating patients, developing and implementing personalized care plans, and coordinating care with an interprofessional team. The nurse supports hospice patients and their families by providing specialized nursing procedures, education, and advocacy to promote patient comfort and dignity.

Key Responsibilities:

• Assess patient needs, preferences, and barriers, respecting cultural, emotional, and spiritual values.
• Develop, document, and communicate individualized care plans with clear goals, ensuring continuity across care settings.
• Promote respectful, empathetic care for diverse populations, practicing collaborative problem solving and advocacy.
• Monitor patient condition changes, inform families and team members, and update care plans accordingly.
• Ensure evidence-based nursing practices to achieve quality outcomes.

Qualifications & Skills:

• Associate’s Degree in Nursing (RN license required).
• Minimum of 2 years of experience; experience in hospice preferred.
• Valid driver’s license and ability to travel within the community.
• Strong assessment, communication, and critical thinking skills.
