Do you love seeing a patient walk out the door to go home? Do you also value the long-term relationships that come from helping residents live as fully and independently as possible? At Birkwood Village and Montrose Health Center, you’ll do both.

We’re looking for a compassionate, skilled Occupational Therapist to join our interdisciplinary therapy team. You’ll work across our full continuum of care:

• Short-Term Rehab: Help patients recovering from surgery, illness, or injury regain the skills and confidence they need to return home safely.
• Long-Term Care: Support residents in keeping their function, comfort, and dignity through meaningful daily activities, positioning, adaptive equipment, and restorative programs.
• Assisted Living: Promote independence, fall prevention, and quality of life for residents who want to stay active and engaged.

What you’ll do:

• Evaluate residents and patients and develop individualized, goal-focused treatment plans
• Provide skilled OT interventions in ADLs, functional mobility, cognition, and safety
• Recommend adaptive equipment and home modifications to support successful discharges
• Collaborate with PT, SLP, nursing, social services, and families
• Complete accurate, timely documentation in line with Medicare and regulatory guidelines
• Supervise COTAs

What we’re looking for:

• Degree from an accredited Occupational Therapy program
• Current Iowa OT license
• Experience in skilled nursing, rehab, or geriatrics preferred; new grads welcome
• A team-player attitude and a genuine heart for older adults
