This is an exciting opportunity for an experienced Registered Nurse who is passionate about delivering high-quality long-term condition management within a supportive and innovative GP practice. Working alongside our GP Partners, Nurse Manager, and multidisciplinary clinical team, you will play a key role in improving outcomes for patients living with respiratory diseases and diabetes whilst contributing to the wider delivery of nursing care across the practice.

The ideal candidate will have experience in respiratory and diabetes care and, whilst an Independent Nurse Prescriber qualification would be advantageous, we welcome applications from experienced non-prescribing Practice Nurses who are keen to develop their skills within a supportive environment.

What You'll Be Doing

  • Delivering high-quality reviews and management of patients with Diabetes, Asthma and COPD
  • Supporting the achievement of Quality and Outcomes Framework (QOF) targets related to long-term conditions
  • Undertaking annual reviews, care planning and patient education
  • Providing health promotion, lifestyle advice and self-management support
  • Monitoring and interpreting clinical results and initiating appropriate follow-up
  • Conducting spirometry (where appropriately trained) and supporting respiratory assessments
  • Delivering insulin initiation support and diabetes monitoring in line with local and national guidance
  • Childhood and adult immunisations, including respiratory vaccination programmes
  • Venepuncture and specimen collection
  • Wound care, dressings, suture and clip removal as required
  • Travel health advice and vaccinations
  • Maintaining accurate clinical records and ensuring compliance with NMC standards
  • Working collaboratively with GPs, HCAs, Clinical Pharmacists and the wider multidisciplinary team to deliver integrated patient care

About YouEssential

  • Registered General Nurse (RGN)
  • Current NMC Registration
  • Experience working within Primary Care, General Practice or Community Nursing
  • Experience managing patients with Diabetes and/or Respiratory Conditions
  • Strong understanding of long-term condition management and evidence-based practice
  • Excellent communication and interpersonal skills
  • Commitment to delivering outstanding patient care
  • Ability to work independently and as part of a multidisciplinary team

Desirable

  • Qualification or recognised training in Diabetes Management
  • Qualification or recognised training in Respiratory Disease Management
  • Spirometry competency and interpretation skills
  • Experience with insulin management and diabetes education
  • Experience using EMIS Web and population health management tools
  • Independent Nurse Prescriber qualification
  • Applications from non-prescribers with relevant primary care experience will also be considered

What We Offer

  • Supportive and welcoming team environment
  • Opportunities for professional development and specialist training
  • Clinical supervision and annual appraisal
  • Protected learning and development opportunities
  • Involvement in service improvement and quality initiatives
  • A role where you can truly make a difference to your community
  • Opportunities to help shape and develop our long-term condition services

Why Join The College Practice?

Whether you are an experienced Independent Prescriber or an accomplished Practice Nurse looking to develop your prescribing skills, this role offers the opportunity to work within a progressive primary care team that values professional development and clinical excellence.
