Description:

The Group Facilitator plans and leads evidence-based psychoeducational and process groups for adults with mental health, substance use, and co-occurring disorders in outpatient and intensive outpatient programs. The role builds a safe, recovery-oriented group environment, documents each session to clinical and payer standards, and works with the treatment team to move clients toward their individualized treatment goals.

Requirements:

Group Facilitation

  • Facilitate groups with typically 8–12 participants, in person.
  • Deliver evidence-based curricula, including CBT, DBT skills, Motivational Interviewing, Seeking Safety, Matrix Model, relapse prevention, and Living in Balance.
  • Lead psychoeducational topics such as coping skills, emotional regulation, healthy relationships, stress management, medication education, and the neuroscience of addiction.
  • Facilitate process groups that encourage peer support, accountability, and safe disclosure.
  • Apply trauma-informed, culturally responsive, and person-centered practices in every session.
  • Manage group dynamics, including conflict, disruptive behavior, and members in acute distress.

Clinical Support and Safety

  • Screen group members for risk (suicidal ideation, intoxication, withdrawal, harm to others) and escalate to the licensed clinician or on-call supervisor per protocol.
  • Follow crisis-response and de-escalation procedures; complete incident reports within [24] hours.
  • Support the use of standardized measures (e.g., PHQ-9, GAD-7, BAM) as directed.
  • Reinforce individual treatment plan goals and relapse-prevention plans within group work.

Documentation and Compliance

  • Complete group notes in the EHR within [24] hours, in DAP/SOAP or program format, linked to each client's treatment plan.
  • Track attendance and participation and report absences and concerns to primary clinicians.
  • Meet HIPAA, 42 CFR Part 2, state licensing, CARF/Joint Commission, and payer documentation standards.
  • Maintain productivity and billing standards set by the program.

Team Collaboration

  • Attend treatment team meetings, case reviews, and clinical supervision.
  • Coordinate with counselors, prescribers, case managers, and peer specialists on client progress.
  • Help develop, review, and update group curricula and materials.
  • Support client orientation, intake into groups, and transitions between levels of care.

Professional Development

  • Complete required trainings (e.g., CPR/First Aid, de-escalation, ethics, cultural competency) and continuing education to keep credentials current.
  • Perform other related duties as assigned.
